But the enactment of Prohibition in 1920 not only made abuse worse (for a number of reasons), but created a crime wave, and essentially created the Cosa Nostra. Because illegality makes any product vastly more expensive than it would be in a free market, some users resort to crime to finance their habits. Because of the risks and artificially reduced supply, the profits to the suppliers are necessarily huge—not the simple businessman’s returns to be had from legal products.
Just as Prohibition of the ’20s turned the Mafia from a small underground group of thugs into big business, the War on Drugs has done precisely the same thing for drug dealers. And is, by far, the major cause of corruption in law enforcement.
The government learned absolutely nothing from the failure of alcohol prohibition. What they’re doing with drugs makes an occasional, trivial problem into a national catastrophe.
Frankly, if you want to worry about drugs, it would be more appropriate to be concerned about the scores of potent psychiatric drugs from Ritalin to Prozac that are actively pushed in the US, often turning users into anything from zombies, to space cadets, to walking time bombs....

In the rush to correct past mishandling of opioid medications, and the understandable urgency to curb an alarming rise in opioid-related overdose and death, the legitimate uses of opioids have been forgotten, as have the people with chronic pain who still rely on them to maintain functional, livable lives.
People with chronic pain need compassionate, calm, rational policies and practices around pain treatment. Above all, they need a collectively strong and respected voice in the ongoing conversation about opioids.
The opioid crisis has always been a real and serious problem. But it has also often been misunderstood, and the response to it mishandled. As medical professionals and lawmakers tighten controls on prescription opioids, people with chronic pain who have genuinely benefitted from them have been not just neglected, but stigmatized and systematically deprived.
“As efforts to address opioid addiction have moved forward,” writes Lopez, “the needs of people with serious pain have been addressed inappropriately or left out entirely.”
“Opioids do carry risks. They are not recommended as a first or second line of treatment for chronic pain for that reason.” However, she also points out that even now, “All current professional and government guidelines provide a role for opioids in pain not managed by other means. For a subset of patients, opioids are beneficial, even indispensable.” Despite popular fear and loathing, there is still a legitimate medical role for opioids.
In recent years, growing concern about the potential dangers of opioids eventually morphed into a full-on social crisis, and calls for an all out war on opioids. Guidelines, regulations, and some laws around opioid use began to be more restrictive. And public shaming of both opioid medications and doctors who prescribed them had an even more punitive effect.
For people who once had a portion of relief from opioid medications, losing access to them is doubly debilitating and frustrating. They suffer more and are less functional not because their underlying medical conditions changed, but because public perception and regulations changed. They are re-inserted into daily agony because of other people’s fear and anger. Even if that fear and anger are justified to some extent, it doesn’t really compensate for the avoidable suffering that results.
