The brass was analyzing where the bullet holes were in returning aircraft and ordering extra armor plating in the areas that suffered the most hits... when in reality they should have been plating the areas where the surviving aircraft weren't hit, since the planes that took hits in those areas, never came back at all.
That is a classic example of the theory.

In layman's terms, it's reacting/acting upon what the winners did while discounting why the losers lost in the first place... which is where the improvement is actually needed.
