This is a regular practice in nursing homes and rehab centers. Medicaid and Medicare pay these facilities according to codes that they can bill for. It's not hard to falsify the paperwork. Some codes pay more than others. The docs and nurses can easily diagnose under vague circumstances. The state will come in to survey their paper trail, but it is impossible to prove wrongdoing. I worked at a hospice decades ago. A bunch of Jews ran it. they had a network of Jewish doctors. Patients are supposed to have a terminal illness and have only 6 months to live. I saw a patient that used to go jogging every morning and mow his lawn. His diagnosis was debility unspecified. Hospice owners love that diagnosis, because of how vague it is. They kept these patients until they squeezed out the last penny from Medicare, then discharge them.
