All it would have required to have universal health care, back in the day, was for government to build hospitals and pay doctors, rather than patients paying doctors.
I think universal health care is the result of extremely sophisticated (digitized) data, which makes triage, logistics and portability possible, as well as higher tax revenue from increased abundance of all commodities due to automation of labor.
Universal health care did not exist prior to the 20th century, (for an entire populace, not just military) anywhere. Why not?
Medicine was extremely primitive up until WWII. We didnt even have penicillin.
