To perform seppuku you need an assistant !
His job, after performing seppuku, is to decapitate the performer with one swing of the best katana available !
If he fails to accomplish his job with one single swing . . . he must himself perform seppuku, with one assistant standing by.
I'm not aware that it ever happened, though I cannot exclude it either . . . but that's the rule !
