I admitted to my doctor I was a smoker and the insurance company saw it on the questionnaire and informed me it wouldn't pay for shoulder surgery.
In order to get coverage I had to quit smoking for 30 days and they tested me to make sure.
I cheated their test but still had to wait 30 extra days to take it. I smoked on the way to the test.
