Medicaid does not directly pay random people to carry your groceries, but it does pay for home care aides and family caregivers who can perform this task as part of their regular duties. Additionally, many specific Medicaid-funded health plans offer pre-loaded cards or grocery delivery services that cover both the food and the delivery service. 
If you or a loved one needs help with grocery shopping and carrying bags, several program options can make this happen. 

1. Self-Directed Personal Care Programs
Most states have Medicaid waiver programs, often called Consumer-Directed Personal Assistance Programs (CDPAP) or Self-Directed Care
  • How it works: Medicaid gives you a budget to hire your own personal care assistant.
  • Who you can hire: You can choose a friend, neighbor, or even a relative (like an adult child) to be your aide.
  • What they do: Medicaid pays this person an hourly wage to help you with "Activities of Daily Living" (ADLs) and instrumental tasks, which explicitly includes grocery shopping, running errands, and carrying/putting away food
2. Dual-Eligible Special Needs Plans (D-SNPs)
If you qualify for both Medicare and Medicaid, you are considered "dual-eligible". 
  • Healthy Food Benefits: Many Medicare Advantage D-SNP plans provide a preloaded "Healthy Food Card" or "Flex Card" worth $25 to over $200 per month. 
  • Delivery Services: These funds can often be linked to delivery platforms like Instacart, which partners with health insurance providers. In this scenario, the delivery driver is effectively being paid through your plan benefits to transport and bring the groceries directly to your door. 
3. State-Specific Medicaid "Food as Medicine" Initiatives
Several states have launched pilot programs—such as CalAIM in California or Section 1115 waivers in other states—that allow Medicaid dollars to be spent on nutrition. This frequently covers the cost of medically tailored meals or healthy grocery boxes delivered straight to your kitchen.
