The 'what is covered' is specific for them, but quite vague for the people. They get a general overview. Vagueness allows them to wiggle out of covering anything.

It's as much of a vicious cycle as what the malpractice award industry has become for we the people versus the Doctors who have needed to jack up medical costs to cover their insurance costs .

My family's Doctor was one of the first wave of doctors to have their careers destroyed by outrageous malpractice awards.
All the great work he did, the late night house calls, the lives he saved as an EMERGENCY DOCTOR [yes family doctors back then were also emergency doctors available at all hours by their clients] was wiped out "just like that".
Good ole Doctor Erlwein did it all...even lab work with a microscope and a centrifuge that he kept in his office.

He was a chain smoker and had bad breath though.
In his waiting room there were ashtrays built into the arm rests of the furniture....
I have a few scars that remind me of the times when the good ole doctor had to sew me back together or else set broken bones.
He made those old fashioned plaster casts in his office too.
