Uh Oh.
Somebody on the Crackerjack Redacter Team forgot to flatten and secure at least one pdf. The copied black redactions pasted into text into a notepad document is removing the rectangles and revealing the information behind them.
You just can't fix stupid. Covering up is not easy.
